The intestinal absorption of vitamin B-12, measured by the double label hepatic uptake test (NAR 37, 3180) in 10 consecutive normal adults and in a patient with pernicious anaemia, decreased when cholestyramine was given by mouth. The mechanism of inhibition was studied in vitro by the guineapig intestinal mucosal homogenate assay (NAR 33, 6047). Cholestyramine reduced the uptake of vitamin B-12 only when allowed to react with gastric juice before the addition of labelled B-12, suggesting that cholestyramine binds a portion of intrinsic factor in gastric juice thereby preventing the formation of intrinsic factor-vitamin B-12 complex. When gastric juice was extracted with cholestyramine, intrinsic factor activity decreased asymptotically as increasing amounts of cholestyramine were used.
