Osteomalacia presenting as chorea

A 16-year-old boy was admitted to hospital after a 2-month history of involuntary movements which lasted for 2 to 6 h. Shortly before admission he had become irrational, lapsing occasionally into a semi-comatose state. Serum was hypocalcaemic and hyperphosphataemic. Improvement in consciousness with cessation of choreiform movements resulted when 10% calcium gluconate was given by vein. Generalised choreiform movements and irritability recurred when serum Ca fell to 6.3 mg/100 ml.
