The case is reported of a 2-year-old boy who presented with otalgia of 3 days duration, in association with a serohaemorrhagic discharge. The child had accompanied his parents while fruit-picking in Extremadura, Spain. Examination revealed the presence of 17 larvae, approximately 8 mm long, in the left ear. The larvae were manually removed. The auditory canal contained a small haematoma and the eardrum had been centrally perforated.
