A study of total LDH [lactate dehydrogenase] and its isoenzymes by polyacrylamide gel electrophoresis in serum and CSF was carried out in 22 inflammatory and 8 noninflammatory [human] cases of CNS disorders to assess the efficiency of the procedure as an early and precise diagnostic tool. The total LDH activity was well correlated with the number of pus cells, but not with severity, mortality and routine biochemical changes in CSF. A characteristic dominance of LDH4 in tuberculous meningitis and LDH3 in pyogenic meningitis in serum and CSF was observed. In viral meningoencephalitis and noninflammatory CNS disorders, the isoenzyme pattern does not deviate significantly from normal pattern. LDH and its isoenzyme pattern may serve as an early diagnostic tool to differentiate various neurological disorders.
