A retrospective study was performed on 1605 patients with histologically proven and radically treated [radiotherapy] nasopharyngeal carcinoma. All were followed for a minimum of 5 yr; 833 patients had a minimum follow-up period of 10 yr. Treatment results were reviewed according to size of primary tumor, base of skull invasion, cranial nerve involvement, cervical node metastases and distant spread. An appropriate staging system was developed that reflected these prognostic factors. In this series of patients, base of skull involvement apparently was less ominous than cranial nerve involvement. Unilteral lymph node involvement carried a better prognosis than bilateral neck nodes; this was the poorest sign of all since it predicted distant metastases. The average 5 yr survival rate for 1605 patients in all stages was 529/1605 (32.1%); the 10 yr survival rate for 833 patients in all stages was 20.2%.
