The pattern of pneumatisation of the normal sphenoid sinus was studied in the skull and paranasal sinus radiographs of 386 African infants and children. The maximum anterior posterior diameter of pneumatised sinus was also measured. There was no significant difference in the degree of pneumatisation between the sexes. Only 20 and 30% of children had pneumatised sinus at 2 and 4 years, respectively. After 4 years, however, there was a dramatic increase in pneumatisation, which was present in all children at 10 years. Only 31.4 and 62.5% basisphenoid pneumatisation was present at 10 and 14 years, respectively. The mean antero-posterior diameter of pneumatised sinus ranged from 13.5 mm at 3 years to a value of 29.2 mm at the age of 15 years. We conclude that the children in this study initially had retarded sphenoid sinus pneumatisation, but attained levels found in caucasians by the age of 10 years. However, they lag behind in basisphenoid pneumatisation even on into adult life.
