We evaluated the gonioscopic findings and pressure-lowering effect of ab externo trabeculotomy in 96 eyes, including 75 eyes with primary open angle glaucoma and 21 eyes with pseudoexfoliation syndrome. The cases were followed up for at least 6 months after surgery. Cases with premature penetration of the probe into the anterior chamber, preoperative peripheral anterior synechia (PAS) or hazy cornea were excluded from the series. Sufficient control of intracular pressure resulted in 43% of the series. The rate of control was lower in eyes with extensive PAS (28%) than in those without (48%). Control of intraocular pressure was relatively poor (25%) in eyes with gonioscopically indistinct cleft. Other gonioscopic features were not correlated with the surgical outcome.
