Pathological features of experimental myocardial ischaemia reperfusion injury

Forty-two dogs divided into 2 experimental groups were used for experimental myocardial ischaemia-reperfusion injury. In the first group, the hearts of 11 dogs were infarcted by ligating the left descending branch of the circumflex coronary artery for 1.0 hour through a left thoracotomy approach. The ligatures were removed after 1.0 hour to reperfuse the hearts for 1 day. Four control dogs had sham thoracotomy incisions. In the second group, the hearts of 16 dogs were infarcted for 1.0 hour as in the first group and the hearts reperfused for 7 days. Two dogs were used as control and had sham incisions. At the end of the reperfusion period, the dogs were euthanized and necropsied. Grossly, the reperfused hearts were rounded and flabby with petecchial haemorrhages, while the control hearts were normal. Histopathological studies of the myocardium revealed oedema of some myocytes, necrotic myocardial fibres, and leukocytic infiltration (mainly neutrophils with few lymphocytes) in the hearts reperfused for 1 day. Sections from the left ventricles of 7 day-reperfused hearts showed degenerative changes in subepicardial zone, uniform necrosis in the subendocardial zone and necrotic papillary muscle. Leukocytic infiltration consisting of neutrophils, lymphocytes and plasma cells were seen in the sections. There was haemorrhage in the necrotized areas. Evidence of similar histopathological change was present in the left ventricles of the hearts outside the risk area. In sections of hearts reperfused for 7 days, there were more necrotic myocardial fibres with loss of nuclei and striations. Monocytes and plasma cells were seen in 7 day reperfused hearts and not in 1 day reperfused hearts. More leukocytic infiltration in 7 days than in 1 day reperfused heart sections were observed. There was no significant histopathological change in any of the control dogs.
