A preliminary study examined the effects of antithrombin III administration in 41 patients with disseminated intravascular coagulation (DIC). Of the 41 patients studied, 28 received an infusion of antithrombin III concentrate in dosages sufficient for maintenance of an internal antithrombin III value of 100%. Antithrombin III concentrate was subsequently administered every 12 hours for the two days. The probable causes of DIC as well as the results of coagulation tests were analyzed for each patient. Patients treated with antithrombin III concentrate showed a tendency toward decreased levels of antithrombin III and platelets as well as elevated levels of D-dimer. Antithrombin III levels and D-dimer levels normalized in five days in those patients treated with antithrombin III concentrate. In the control group, however, elevated levels of D-dimer were interpreted as an expression of intravascular fibrin formation and secondary fibrinolysis. Approximately half of the patients in each group showed improvement. A survival rate of 70% after 15 days was noted in both groups.
