In order to improve the shape of a reconstructed auricle due to microtia, the authors have performed operations utilizing several techniques that follow. The most depressed area, determined by having the patient open his mouth, was found to be the central portion of the conchal cavity which had to be created. Conchal part of the contour pattern was superimposed on to the selected conchal area, after which the lobule position, which is usually anterior and inferior to the lower part of existing lobule, was determined. Transfer of the existing lobule to the determined position was performed by using a subcutaneous pedicle flap. Subcutaneous tissue flaps were hinged at the conchal area to deepen the conchal cavity and to cover the exposed portion of the cartilage framework for a free skin graft. This causes the skin flap on the framework to loosen. A large triangular flap is raised and wrapped about the cartilage block to make a tragus. When the cartilage framework is completed, a thick strong base, a high helix, a high antihelix and a stressed antitragus should then be created. During the past 10 years, 22 cases of microtia, including 13 completed cases were treated in this manner with good results. Details of these methods are reported and discussed.
