We immunohistochemically examined myofibroblasts for the expression of fibronectin, proliferating cell nuclear antigen (PCNA), and adhesion molecules at the two stages of healing in human gastric ulcers and in acetic acid-induced ulcers in young and old rats. Actin-positive myofibroblasts were seen at the periphery and floor of the ulcer in the initial healing stage (IHS) in patients and young rats. Intercellular adhesion molecule 1 (ICAM-1)-positive cells were also seen at the periphery of the ulcer in the IHS in human gastric ulcers. In the proliferative healing stage, strongly actin-positive deposits were observed within the cytoplasm of the myofibroblasts and in rows on the floor of the ulcer. Such deposits were not seen at this stage at the periphery of the ulcer in the patients or in young rats. In the IHS, epithelia for strongly positive PCNA were seen at the periphery of the ulcer, and fibronectin-positive netlike deposits were observed in the floor of the ulcer in this stage. The frequency of the actin-positive myofibroblasts was decreased in the IHS in old rats. Thus, it appears that actin-positive myofibroblasts are responsible for the reduction in area observed clinically in the IHS in gastric ulcers, and the frequency of these cells decrease with age.
