Pineocytoma is considered to be a neoplasm of pineal parenchymal cell origin. We encountered a case of pineocytoma with pure neuronal differentiation in a 55-year-old woman. Under light microscopy, the tumor cells were arranged in an incomplete lobular pattern. The tumor was mainly composed of small round or oval cells with occasional large cells. In some areas, tumor cells formed pineocytomatous rosettes. Immunohistochemical staining demonstrated a positive reaction for synaptophysin, and a negative one for neurofilament. NSE, GFAP and S-100 protein. Ultrastructurally, most tumor cells were small and cytoplasm contained aggregates of clear vesicles, mitochondria, free ribosomes, intermediate filaments and dense-cored vesicles. Clusters of dense-cored vesicles were occasionally observed in the cell processes. In the large cells, aggregates of clear vesicles, dense-cored vesicles, mitochondria and free ribosomes were constantly present. Synapse-like complexes were occasionally seen. These findings confirmed that most tumor cells showed neuronal differentiation. Electron microscopic examination would appear to be a valuable method for the diagnosis of pineocytoma and elucidating its degree of differentiation.
