The authors reported a case of 75-year-old man who had pulmonary artery thromboembolism after femoral head endoprosthesis, resulted in acute respiratory distress. He had a left femoral neck fracture by a fall during hospitalization for the treatment of his heart disease. He were underwent femoral head endoprosthesis by using bone cement under spinal anesthesia. After one hour of operation, a severe respiratory distress, a drop of blood pressure were observed. Despite of immediate resuscitation, he had cardiorespiratory arrest, resulted in acute death. At autopsy a massive fresh thromboembolus was found in the trunk of right pulmonary artery. During under microscopic examination erythropoietic cells in fatty tissue were identified in the thromboembolus. These findings suggested that femoral bone marrow tissue spreading at operation induced the thromboembolism in the pulmonary artery. For the prevention of the pulmonary artery thromboembolism, examination of medical specialist were important pre- and post-operatively.
