A diabetic patient complicated by septic pulmonary embolism

We described here a rare case of diabetes complicated by septic pulmonary embolism. A 56-year-old male, who had been diagnosed as having diabetes mellitus in 1981, was hospitalized with a urinary tract infection (UTI), Laboratory examination on admission revealed leukocytosis and a high level of serum CRP. The antibiotic piperacillin sodium (PIPC) was administered. Five days later, chest roentgenography showed a nodular lesion in the left lung field. Nodular lesions were detected in both lung fields by chest computed tomography. In the urine and blood cultures, staphylococcus aureus was isolated. Therefore, we diagnosed septic pulmonary embolism occurring secondary to UTI. Although panipenem/betamipron (PAPM/BP), which shows sensitivity to staphylococcus aureus, was used, it was ineffective in this case. Amikacin sulfate (AMK) was then administered and was effective. Three days after the start of AMK therapy, the patient's fever alleviated. Furthermore, the nodular lesions shown by roentgenography and computed tomography improved.
