Hemovigilance has become an integral part of the transfusional safety. Besides donor selection, control of the manufacturing process and the use of inactivation procedures, the tracing of stable blood products (SBP) contributes to improve the safety of plasma derivatives. French legislation specifies that the pharmacist has the responsibility for the link of the information between the SBP and the patient. Thus, the computerization of the prescription book of SBP at the department of Pharmacy of the Teaching Hospital Trousseau in Tours (France) allowed us to measure the tracing ratio of SBP about 96,5% for 1997. Defaults in tracing SBP are particularly due to lack of exhaustive notifications on prescription forms by the responsible care unit. Both pharmacist and physician must be aware of the absolute necessity of checking and keeping all legal informations.
