This paper reports a case of West Nile virus (WNV)-associated meningoencephalitis in an 8-year-old boy from Maryland, USA [date not given]. The patient's clinical course was complicated by myocarditis, quadriparesis and respiratory failure. Polymerase chain reaction and immunological results confirmed the diagnosis of WNV infection. The patient was treated with intravenous anti-WNV antibodies (4 doses of 0.5 g/kg). At 2 months post-admission, the patient remained ventilator-dependent with minimal additional improvement. This case represents, to our knowledge, the most severe manifestation of WNV infection in a surviving child reported in the English medical literature. It illustrates that children can suffer the same devastating, long-term paralytic manifestations as in adults.
