The non-specific interstitial pneumonitis (NSIP) is an important differential diagnosis of extrinsic allergic alveolitis (EAA). While the acute course of EAA in spite of similar bronchoalveolar lavage and radiological patterns can be clearly differentiated by serum antibodies and the temporal relationship between antigen contact and clinical symptoms, the chronic course of EAA is difficult to differentiate. Even by open lung biopsy a definite diagnosis can be established in all cases. Due to possible long-term consequences (antigen avoidance) however, an open lung biopsy should be performed in questionable cases.
