A patient with chronic ulcerative colitis and severe liver disease has been observed for a prolonged period. The hepatic disease progressed to biliary cirrhosis despite control of the colitis symptoms, antibiotics, and colectomy; but responded favorably to treatment with adrenocortical hormones. His course was complicated by a Coombs-positive hemolytic anemia. He had a markedly elevated serum concentration of alkaline phosphatase and 5-nucleotidase. Simultaneous determination of these enzymes in blood drawn from the mesenteric veins, hepatic and peripheral veins indicated that the liver was the source of the enzymes.
