Comparison of chlorpropamide and metformin treatment on weight and blood-glucose response of uncontrolled obese diabetics

In a prospective study 77 obese diabetics, uncontrolled by diet, completed one year's satisfactory treatment with chlorpropamide and with metformin, each being preceded by a period on diet alone. Irrespective of the sequence of therapy most gained weight when taking chlorpropamide; the mean increase at 12 months was 11.6 lb., there being statistical evidence of continuing weight increase throughout the period of treatment. In contrast there was a mean weight loss of 2.7 lb. after 12 months' metformin treatment but no significant mean weight loss during the 2nd 6 months of therapy. The weight loss may be caused by a reduction in food intake during diguanide treatment, or by some non-tissue-forming catabolic process. Increasing weight in an already obese diabetic is clearly undesirable, and it is recommended that the obese, overeating, insulin-uncontrolled, independent diabetic should be treated with a diguanide.
