The electrocardiographic changes in 370 patients, who underwent mitral commissurotomy are described. In 204 instances the period of follow up was 2 to 9 years and the results were compared with preoperative ecg examinations. In 44.7% of all abnormal preoperative ecg tracings full normalization of the electrocardiogram has been found, in a greater percent among patients operated in an early group of mitral stenosis. In 1/3 of this group it was pseudo-normalization of the electrocardiogram. The reason for this was creation or augmentation of mitral regurgitation during valvulotomy. The problem of atrial hypertrophy was analyzed. Careful examination of tracings was done in cases where ventricular complex revealed abnormal deflection of the QRS-T gradient or hypertrophy of right ventricule. It is emphasized that abnormal ventricular gradient is an earliest ecg pattern of righ ventricular hypertrophy.
