In hearts of 18 open chest dogs anesthetized with sodium pentobarbital, local ischemia was produced by occlusion, ligature or perfusion stop of the anterior descending branch of the left coronary artery. In 6 animals, the descending branch was cannulated and perfused with constant volume. Myocardial pressure was measured within the distribution area of the occluded artery. With the onset of ischemia, the systolic myocardial pressure decreases immediately. It disappears completely within a period varying from 15 sec. to 14 min. depending on collateral arteries and on the position of the recording cannula. The recovery is complete after ischemia of short duration. The observation that systolic myocardial pressure disappears completely demonstrates not only that it depends on the local contraction but also that the intraventricular pressure decays within the innermost layers and is not transmitted evenly through the wall of the Infarcted area. This may be due to the bulging of the infarcted area. Increasing the perfusion rate causes an increase of systolic and diastolic myocardial pressure. At a perfusion rate higher than the double spontaneous blood flow, a further elevation of diastolic and a decrease of systolic myocardial pressure are observed. These changes are believed to be due to the development of myocardial edema.
