Micro-electrode recordings were undertaken in eeg spike foci to establish the basic features of single neuron activity in human focal epileptogenic cortex. Records were obtained from over 100 single neurons in 28 craniotomies in patients with drug-refractory focal epilepsy. During simultaneous pial and intra-cortical recording of spontaneously occurring epileptifom spikes, a greater complexity of intracortical slow wave and unit activity was found in man than in similar records from experimental epileptiform spikes in monkey and cat. Two distinctive patterns of intracortical potential configuration were recognized which are not reflected in the surface spike. In the Type A pattern the surface-depth relationship was relatively simple, suggesting activation in the superficial portions of radial cortical elements. The cortical units fired without preeminent relationship to any one component of the slow potential. The Type B pattern consisted of a multiphasic intracortical slow potential resembling a spike-and-wave complex, the deep-negative sharp wave occurring simultaneously with the small initial positivity of the surface spike. Cortical units were characterized by a high probability of activation during the rising phase and the peak of the deep-negative sharp potential. Some epileptiform spikes were probably generated with the participation of normal cortical mechanisms. The discharge parameters of 30 of the best analyzed units have been examined with the aid of a computer. Unit discharges during and between epileptiform surface spiking occurred with a similar spectrum of short intervals. Some increase in the incidence of short intervals corresponded to the increased rate of unit firing during surface spiking. Epileptiform surface potentials in human cortical foci are associated with single unit discharge patterns which offer no evidence of intrinsic neuronal abnormality.
