Valve replacement in cases of aortic insufficiency due to active endocarditis

Aortic valvar insufficiency due to acute bacterial endocarditis may cause fatal heart failure before adequate antibacterial treatment can be given. While it might be desirable to await bacteriologic sterilization, this is not always possible. It is advisable to replace the valve in the early phase of infection, before uncontrollable heart failure precludes survival. The feasibility of this course of action is demonstrated by the results in 12 collected cases, 3 of our own and 9 reported by others. Eight of these 12 patients are alive at the time of report, despite extension of the infection to the annulus or aortic wall in some instances. Operative indications are reviewed in the hope that wider use of early operation will improve the serious prognosis in this condition.
