Background: Outpatient UTI's are common. However, antibiotic resistance is increasing to beta-lactams and sulfonamides. The purpose of this study was to assess the prevalence of antibiotic resistance in outpatient urinary isolates from North America. Methods: 30 medical centres representing all geographic regions of the United States (US) Bureau of the Census and 10 medical centres from all regions of Canada (CAN) submitted up to 50 consecutive outpatient urinary isolates to the coordinating reference lab (Health Sciences Centre, Winnipeg, Canada). NCCLS-specified MIC microdilution broth testing was performed with ampicillin (Amp, range 0.5-2048 ug/ml), trimethoprim/sulfamethoxazole (TMP/SMX, range 0.25-1024 ug/ml (TMP)), nitrofurantoin (Nitro, 2-1024 ug/ml) and levofloxacin (Levo, range 0.03-32 ug/ml). Results: Species prevalence of the 2000 isolates received and tested included: E. coli 57.1%, Klebsiella pneumoniae 12.5%, Enterococcus spp. 7.1%, Proteus mirabilis 5.0% and Pseudomonas aeruginosa 2.9%. Antibiotic resistance rates (%) among all isolates tested (n=2000) and E. coli only (n=1142), respectively, are listed. Conclusion: Antibiotic resistance in outpatient urinary isolates obtained from medical centres in the US and Canada against all isolates and E. coli only, respectively, was Amp 42.8% and 38.4%, TMP/SMX 20.9% and 22.2%, Nitro 10.5% and 1.5%, and Levo 8.2% and 5.8%.
