Patients receiving a liver graft need to be treated with immunosuppressive drugs to avoid rejection. These kinds of drugs predispose to the reactivation of latent infections such as tuberculosis. Therefore, it is necessary to establish treatment regimens to avoid them. We retrospectively analyse all the patients receiving a consecutive liver transplantation in our center since 1st January 2 until 31st December 2In all of them, latent tuberculosis infection (LTBI) was diagnosed by positive tuberculin skin test. After liver transplantation, they were treated with isoniazid for six months, starting on the first postoperative period if any of them were positive and then follow up until the end of the study. During this period 53 patients had data of LTBI. All of them were treated with isoniazid after liver transplantation. The median observation period after liver transplantation was 52 months (range 12?129). No case of reactivation of tuberculosis was reported during follow up. Only 4 patients presented altered liver enzymes related to this treatment, with clear improvement after it ceased. None of them showed severe dysfunction of the graft. Preventive isoniazid would appear to be a safe drug for use in LTBI after liver transplantation. The treatment may be established just after liver transplantation without an important dysfunction of the graft or severe consequences to the patient. Liver Transpl, 212.
