Since it is well recognized that providing quality services is the key to creating effective demand for family planning, serious efforts are being made to qualitatively improve family planning services in India. Launched in 1951, India's Family Welfare Program has evolved into a cafeteria approach. The program provides family planning services through the primary health care infrastructure of more than 2000 community health centers, 21,000 primary health centers, and 140,000 sub-centers emphasizing voluntarism and an informed choice from among a range of contraceptive methods. The network covers an average population of 304,000 per community health center, 30,000 per primary health center, and 4800 per sub-center. With outreach improved, government attention has shifted to improving the quality of family welfare services. India's 8th five-year plan reflects this concern and provides for the consolidation rather than expansion of infrastructure. The differential approach demanded by the demographic and health profile diversity in the country has been institutionalized by policy since 1992 and should help direct resources and focus attention in the lagging areas. The upgrading of physical infrastructure and quality control of family planning and maternal-child health supplies are discussed.
